The case scenario is about a patient that was admitted into a hospital ward with a sudden stroke condition . A student nurse on placement in the hospital developed a good communication relationship with the patient and during a placement shift, the patient had an episode of distress situation where he was very verbal , abusive and physical. The student used a good communication skill to calm the patient down and engaged in a caring and conversation discussion , the patient narrated his health ordeal and how his mental distress which started from his failed marriage led to a spiralled mental breakdown. He neglected doctors advices and failed to take his medications, depression and excessive thinking contributed to a severe hypertensive condition that caused him the sudden stroke.

Write an essay of at least 750 words (including quotations from the poems and prose selections that you use as support) on ONE of the general topics below.  You must discuss at least three reading selections for the topic you choose.
If you use secondary sources in your essay (even those I have provided for you), be sure to cite them, using MLA format. Even if you don’t quote directly, you must cite secondary sources in the paper itself. You will also need to include a works cited list.
Quotations from the textbook require only a page number. Do not repeat the author’s name in parentheses. Just put the author’s name in a sentence in your discussion.
You should choose different poems or prose selections (unless the topic states otherwise) than those you used in your reading responses.
The three examples should be by different authors.
Some of the topics focus on Victorian authors, though for others, you may wish to include writers from the Romantic age.
The paper should be in essay form with an introduction, thesis statement, body paragraphs with transitions, and a conclusion, not a collection of reading responses with sub-headings.
You must use quotations from the reading selections as support for your general ideas. Your examples must show that you have read the literature that you are discussing, not just the author’s biography or an online summary.
A brief overview isn’t adequate. Again, you must show that you read the selection (poem or prose selection) carefully, and you must show how the body paragraphs support your thesis.
You should not choose long poems or plays, since it would be difficult to discuss these thoroughly in a short paper. (You will have an opportunity to write about them in the research paper.)

Chief Complaint: 62-year-old man with esophageal bleeding
History: Vincent Miller, a 62-year-old accountant, has had a “drinking problem” throughout most of his adult life. He drinks about a half-case of beer each day. He has lost several jobs over the years for drinking at the workplace or showing up for work drunk. He lost his driver’s license for drunk-driving, and his drinking has placed a considerable strain on his marriage. He has tried several self-help programs as well as Alcoholic’s Anonymous, all with little success. He has been hospitalized on several occasions over the years. Vincent has a severe tremor in his hands (probably a result of excessive alcohol intake), which makes it very difficult for him to use a spoon, fork, and knife to eat. It’s your first day on the job as an occupational therapist, and you are consulted by his physician to see if there is any way to help Vincent use eating utensils. Not knowing anything about him, you open up his past medical records, which, incidentally, are quite thick.

First Hospitalization:
You note that Vincent was hospitalized at age 32 with a complaint of vomiting up blood after a drinking binge that lasted seven days and was marked by excessive and repeated vomiting episodes. The vomitus was bright red.

The hospital chart lists a diagnosis of “Upper GI bleed” due to a Mallory-Weiss tear. You look up “Mallory-Weiss tear” in an internal medicine textbook and see that it is defined as “a longitudinal tear in the mucosa at the gastroesophageal junction — i.e. in the area of the lower esophageal sphincter — caused by repeated vomiting.”

Second Hospitalization
At age 36, Vincent was hospitalized again, this time with complaints of abdominal pain in the upper epigastric region (i.e. just below the xiphoid process of the sternum) and “coffee-grounds” emesis. He also complained of “heartburn” (a burning sensation in the area of the sternum) which was partially relieved with antacids. A diagnosis of “upper GI bleed due to gastritis and reflux esophagitis” is noted in the chart.

It is an unfortunate fact that health disparities have a great impact on the health of minority populations. This is especially prevalent in latino/hispanic patients with diabetes. Diabetes is an epidemic among all ethnic backgrounds in the United States, but certain minorites have been found to have much larger prevalence then others. According to the American Diabetes Association (2017), Hispanics/Latinos are 70% more likely to be diagnosed with diabetes then non-hispanic caucasians, and have a higher risk for complications such as kidney failure and heart disease. Spanakis and Golden (2013), stated that although the CDC estimates that 11.8% of Hispanic Americans are diagnosed with diabetes, the Hispanic Community Health Study/Study of Latinos raised that estimated prevalence to 16.7% in men and 17.2% in women. They also stated that Hispanic/Latinos are 1.5 times more likely die from diabetes (Spanakis & Golden, 2013). Considering that Hispanic/Latino Americans are the largest ethnic minority in the United States, these percentages represent a large and growing problem that needs to be addressed. These discrepancies are due to a variety of factors including biological, behavorial, social, environmental, socioeconomic, language barriers, and health care system problems. Although some contributing factors, such as biologic or genetic, are difficult to change, most studies agreed that a focused community centered approach could help mitigate many of the problems. The first steps to implementing any changes are always participation and education. To increase the amount of the Hispanic/Latino population receiving the neccesary education, the solution is to bring it to them in their commmunities. One article by King, Fleck, Estrella, and Reitz (2013), discusses the implementation of a community based diabetes self-management program by the Centers for Medicare and Medicaid Services (CMS). The program, called Every Diabetic counts, was meant to recruit providers and medicare participants, increase ordering and taking of tests, provide self-management education, and improve outcomes (King et al., 2013). The programs were community based and administered by community healthcare workers and physicians (King et al., 2013). The program was overall pretty successful, but ran into time and funding based barriers, and required a copay that could be prohibitive (King et al., 2013). Another study more specific to the Hispanic/Latino population was conducted by Castillo et al. (2010), and was another community based education plan. The program had community health care workers recruit Hispanic/Latino with type 2 diabetes and provided them with 2-hour classes over 10 weeks (Castillo et al. 2010). The results showed improvements in A1C levels, systolic blood pressure, diabetes knowledged, physical activity, diet, glucose monitoring, and medication adherence (Castillo et al. 2010). The key was bringing the education to non-clinical locations in their community, tailoring care to the individuals, and training spanishing speaking community healthcare workers to administer the education (Castillo et al. 2010). As a primary care provider, I can become part of this change in several ways both in my practice and my community. One is employing translations services through staff or translation lines to help break down communication barriers. Secondly I can provide nutritional consults for patients to tailor dietary plans to each individual. This can be accomplished through the PCP (myself), a staff nutritionist, or a referral depending on insurance and needs of the patient. Make sure my patients will be able to get the medications I am prescribing and understand the proper medication administration. I can also provide handouts with diabetes education and a list of community resources for my patients to reinforce the education I am giving them. By individualizing the care of all of my patients, hopefully I can help overcome some of the barriers to healthy outcomes. On a community level, I can become an active particpant in community diabetes education programs and advocate for continued research and policy changes. It is through the growth of participation and support for these programs, both by research and policy, that the health discrepancies in Hispanic/Latino diabetics can be decreased.

The first step of the EBP process is to develop a question from the nursing practice problem of interest. Select a practice problem of interest to use as the focus of your research. Start with the patient and identify the clinical problems or issues that arise from clinical care. Following the PICOT format, write a PICOT statement in your selected practice problem area of interest, which is applicable to your proposed capstone project. The PICOT statement will provide a framework for your capstone project (the project students must complete during their final course in the RN-BSN program of study). Conduct a literature search to locate research articles focused on your selected practice problem of interest. This literature search should include both quantitative and qualitative peer-reviewed research articles to support your practice problem.
